My rulebook is officially found now (on the kitchen floor...I suspect either gremlins or the two-year-old). And, yeah, reading through again, I can't find the part I'm looking for. So I would now say that I agree that it reads the way you're interpreting it. I suspect that this is an omission, because it leads to some problems in use. For instance, by this reading, if I'm at -50%, and I get a -10% result, since that's associated with the new state that the rules say my character is in, I could assume that the penalty is reduced to the level of the new state. Which is an interesting notion, but which I rather think they weren't intending.

As you point out the rules do explicitly say that one can have multiple hurts, so that implies a cumulative effect.

As for my mystery clarification text, I suspect that this was in Hero Wars. I do recall that where ever it is, it was in a very non-intuitive place (not with the rules on injuries or healing, but more like the narrator advice section or something).

In any case, since the rules don't seem to be helping much, and since "injuries" really just mean "penalties applied according to the specific results of contests," I'd advocate thinking of each negative consequence of a contest as a separate effect. No different than other situational modifiers that reduce the TN of the character (simply handled differently, given that they're percentages instead of flat penalties.
