Not a member of the Moonies, but the survey will come out later. They've explained to the Kickstarter supporters that all the new material being added due to goals-met, means the books will come out now around MAY.

They are holding off on the survey, as Kickstarter only allows them to send out ONE, and they want to make sure that they cover everything they need to cover at the last possible moment.
