Defenses do not reduce target numbers. The higher the defense's target number, the more likely the defender is to successfully defend so your idea is right on. Also, it could be used as the defending skill. For example, if Bob is throwing his spear at Jim, Jim could use his Dodge 2W as the resisting ability to Bob's Ranged Combat (Throw Spear) 3W. Or if Bob was trying to stab at Jim using his Close Combat (Spear and Shield) 5W, Jim could use *his* Close Combat (Spear and Shield) 4W augmented by his Dodge 2W (and probably get a boost to his CC of +2 or more.)

The way the rules are currently written, no. Some people (including me) like the idea of certain "always on" physical abilities such as Large.

One of the things that is very different about HW from other RPGs is that when you're "attacking" an opponent, it's not a simple hack with a sword or stab with a spear but a full exchange. That's one of the reasons that you can lose AP on an exchange you initiate. If you make a bid and roll and the result is that you lose AP, it could be that the opponent ended up with a successful riposte, knocked you off balance, or just plain made you look bad.
