I agree, the more I think about this the more I like it. Some playtesting would be needed but the fundamental idea is so clean and fits so seamlessly with the game system that it just feels right.

I think I would go with simply replacing the 7AP per hurt rule with a new ability at level equal to the AP. There may be balance issues but for the sheer narrative advantages I think it would be worth the effort required to iron them out. Applying the consequences table guidelines the wounds dished out by hero level characters[1] would often leave even the winner of a contest with long term or perhaps permanent consequences.

With a bit of imagination they could have wider uses, perhaps using that nasty wound to augment Boasting after the fight is over. Or to demonstrate to your patron how tough a job it was and how dedicated you were to carry on (hoping for greater reward/recognition). Or to elicit sympathy from that healer the character always rather fancied...

Perhaps some healing only changes the description of abilities - from wound to scar. Then you can re-enact that comparing-the-scars scene from Jaws with your favourite HQ characters.

[1] I am assuming that AP bids are proportionate to the total AP of the hero, a 10w3 hero bidding 5AP is really not trying. A bid of 20-25 would be more typical and at that level you could start to seriously hamper your opponent. IIRC there is a description of Harrek vs JarEel that had the winner left with a number of "permanent" crippling injuries.
