This, at least, is a problem a GM can easily take care of with the HW rules. Assign a flaw of some sort for the first kill and add to it or add others depending upon the circumstances of the killing. A desperate struggle for survival leaves an impress on one -- and so does slaughtering weak opponents -- just different ones.
