Vague memories just came back of a trilogy where this happened, thogh sadly I can't remember the details.
Damiano's Lute. The hero dies of plague at some point quite early in the series, but continues to interact with the rest of the party. Sorry, it was twenty-odd years ago when I last looked at it, so that's about all I remember :(
