Some thoughts and suggestions:

As far as I know Carmanians aren't as obsessed with Chaos as Orlanthi. A Chaos taint is not impossible but I would probably not use it. Instead the child might be monstrous as a manifestation of The Lie, perhaps with Darkness associations. (If cannibalism isn't too strong for your game that might fit with Darkness elements)

A witch in Carmanian terms would probably be following the old Spolite Darkness worship. I supposethat some Pelnadan religions - those that are not adopted by Carmanians - might be considered "witchcraft".

Perhaps it was not one child, but twins. One weak but "good", one monstrously deformed. (Fits in nicely with Carmanian Dualism). Either would probably have been killed, if they had been left.

Perhaps the child was born "dead" but the midwife "saved" him with unfortunate side-effects.

Could be. Perhaps the son has a dual nature - something like a werewolf - and the "good" side has to be strengthened somehow. Perhaps part of that is for all those involved to tell the truth.

Could be. If you go with the midwife saving the child by magic, perhaps the price is that the Darkness spirit that save the child possesses him - perhaps only occasionally, perhaps all of the time.

As a variant of the twins idea, perhaps a monstrous spirit was able to manifest as a result of the lies surrounding the birth. Perhaps killing the son is the easiest way of destroying the spirit - but there may be a more difficult way that permits the son to survive.
