This is not a 75% chance of success, it is a 95% of success. It is a 75% of Complete Success which results in double the target augmentation. With a w2 advantage over the target augment there is almost no chance of failure.

Never seen one. We have seen minor defeat when the GM rolls a better success than the player does. That results in a penalty rather than an augment. We've never seen a failed or fumbled roll, because the players always have a W or W2 advantage over the target.
