Elmal is for the very few Orlanthi who are strongest in the Fire Rune. Heler is a special case and can be worshipped both as a subcult of Orlanth (Air with power over Water) or - less commonly - as an independent god (Water with connections to Air).

BTW, keep in mind with the above that terms like "subcult" or "independent" god are artificial games-rule conveniences.
